Effect of Esmolol on Induced Hypotension with nitroglycerin in Rabbits


Abstract
The purpose of induced hypotension is to decrease blood loss, thereby improving operating field and decreasing the need for blood transfusion and the potential for tranmitting disease. It is achieved by decreasing arterial blood pressure to
25-30%
of
mean arterial pressure.
Several drugs such as nitroprusside, nitroglycerin (NTG) are used for induced hypotension. Although NTG induced hypotension is weak, NTG is commonly used because of several advantages.
For the purpose of reducing dose requirment and complications of NTG, ¥â-adrenergic blockade is used.
Esmolol, short acting ¥â-blocker, is cardioselective with an elimination half life of 9 minutes and rapid onset, thus theoretically making it an ideal agent for potentiation of NTG induced hypotension.
This study was performed to evaluate the effect of esmolol on hemodynamics, plasma renin activity and catecholamine in NTG induced hypotension.
Thirty healthy rabbits, weighing 2.0-2.-kg, were divided into theree groups.
NTG was infused with 3-§¶/kg/min intravenously in NTG group. NTG was intravenously infused with 30§¶/kg/min and esmolol with 300§¶/kg/min simultaneously in NTG combined with esmolol group. After intravenous bolus injection of 500§¶ esmolol,
esmolol
was
infused with 300§¶/kg/min in esmolol group
Blood pressure and heart rate were measured at pretreatment, 10, 20 minutes after infusion of drug and 10 minutes after the cessation of infusion of drug in all groups. At the same time, blood samples were withdrawn for the estimation of plasma
renin
activity, epinephrine and norepinephrine level.
@ES The results were as follows:
@EN 1. Mean blood pressure was significantly decreased to 27.6% at 20 minutes after infusion of drug in NTG combined with esmolol group, but was not decreased in NTG and esmolol group.
2. Heart rate was significantly decreased from 249.0¡¾6.3 to 202.4¡¾8.9 beats/min at 20 minutes after infusion of drug in esmolol group and from 252.4¡¾5.6 to 212.1¡¾24.2 beats/min in NTG combined with esmolol group.
3. Plasma renin activity was decreased in NTG combined with esmolol group compared with NTG group.
4. plasma epinephrine was significantly decreased in esmolol group compared with NTG group.
5. Plasma norepinephrine was increased in NTG combined with esmolol group.
These results suggest that esmolol enhance NTG induced hypotension and decrease the compensatory hormone response.
